I have done it over the years in the following format:
on the face
- translation and affidavit, my certified translator stamp and signature.
on the overleaf
- true copy of the original.
The stamp contains my name, certification body and my certification number, and my complete adress.
The signature is my usual illegibile signature, NOT "my name written in my own handwriting."

Recently I have a request to remove the true copy of the original,

I would like to know if there is a protocol for a translation of a legal document.

Who made the request? Your procedure sounds quite reasonable. And assuming your name appears in print, the fact that your signature is illegible (mine is too) should be irrelevant. What matters that it is your usual signature.

So I guess you need to address your question to the requestor.
