It is a Polish surname, not very common, though. It should be pronounced like ko-ra-chick with the stress falling on "ra".

コラツィク or コラチク
This looks a name of Slavic languages (i.e., Russian, Czech, Romanian, Slovene (or Slovenian), etc.). In these languages, "y" is pronounced like "イ", and "cz" is pronounced like "チェ" or "チュ". ("チェ" is close but not precise, and the English "ch" sound is a bit closer than "チェ".) So, "コラツィク" or "コラチク" will be closer pronunciations. I think "c" will not be pronounced separately.
An American phonologist at MIT, Michael Kenstowicz, is "マイケル・ケンストヴィツ" or "マイケル・ケンストヴィッツ". So, "cz" will be something between "ツ" and "チュ".
