"i perioder" This is a very common expression, and not necessarily a fancy medical term. It means more something like "occasionally" or "at times", and "at the time"

I'd say that "i perioder" in this context means "episodically" and that the second instance refers to the specific period, so perhaps "during this period" or "at this time".
