The context in which this term appears is a lengthy presentation of hypothetical scenarios over the course of two closely printed pages of an insurance policy for a medical supply company. These hypothetical scenarios are designed to clarify to which insurer a claim should be presented in particular circumstances in which it might seem plausible that two different insurers could be asked to honor a given claim. Thus:

: That's the literal translation, but I still don't understand what "cuando su contrato se active" means.

My reading of 'cuando su contrato se active sobre la base de': refers to the insured making use of (i.e., recurrir a) his/her contract (policy) by reason of--in response to--the event in which s/he sustained damages. The reflexive/impersonal is a bit odd.

But I ended up translating it along the lines PJ suggests and I appreciate his attempt to help. The Spanish here does at the very least here seem to be poorly drafted....
