****El aspecto biomicroscópico característico en un ojo sintomático*** es un agujero de 400-600 micras de diametro, redondo u oval, con bordes bien definidos, en el centro de la mácula, rodeado por un halo grisáceo de engrosamiento de la retina. Es relativamente frecuente el adelgazamiento o atrofia del epitelio pigmentario en el área del agujero. En un 50% de los casos se observan opacidades nodulares amarillentas encima del epitelio pigmentario del agujero, y pueden aparecer rápidamente, incluso a la semana de haberse formado el agujero (fig. 1). Estas opacidades cambian en número y distribución de un examen a otro. El epitelio pigmentario y coroides adyacente al agujero permanece normal excepto en algunos pacientes mayores que pueden presentar drusas alrededor del agujero. En ocasiones se observan multiples quistes en la retina adyacente al agujero (fig. 2). Estudios histopatológicos postmortem en ojos afectos de agujero macular revelan una alta incidencia de edema retiniano quístico periagujero (68%), asi como presencia de membrana epirretiniana macular.
