acute-on-chronic kidney injury
Two things to note:
1.The old tern "kidney failure" has been changed into "kidney injury";
2. Acute-on-chronic indicates acute exacerbation of chronic kidney disease -- think of it as acute injury superimposed on chronic kidney disease.

: I agree with "acute-on-chronic", but IMO it's more appropriate to follow ICD-10, which doesn't yet mention "injury". Standardization of terms is crucial, which is why the ICD was developed.

I beg to differ. Don't let the word "injury" (in a purely linguistic way) bother you.The underlying mechanism is not imprtant, it just means that the kidneys stop working suddenly. In the past, it was called "failure", now they've changed it into"injury".

"acute ON chronic kidney/renal failure" seems to be the usual way to express it.

Portuguese term (edited):
insuficiência crônica renal agudizada
aggravated chronic kidney disease

: Aggravated is not accurate medical terminology, it is not idiomatic. Borrowed from the legal field: aggravated assault??

: I posted the same answer and then hid it. Lirka is correct about "acute-on-chronic." That's the term in the International Classification of Diseases. However, the ICD says "renal failure'.

Based on Lirka's answer, I checked ICD-10--see below. Either of those answers should be OK, but the ICD should be followed, and it doesn't mention "injury."
CD-10-CM Diagnosis Code N18.9
Chronic kidney disease, unspecified
Acute with chronic renal failure; Acute-on-chronic renal failure;

: "failure" is the same as injury (see ref.), injury is just newer. It is not the term I invented, it does sound a bit odd to me to. but it's the new terminology (official). But failure would pass, too, it's not wrong.
