It is assumed that a psychiatrist provides the treatment for attention deficit disorder. The AR-therapist can support attention deficit disorder treatment; however, in consultation with the psychiatrist.
The following topics should be discussed:
1. What does stand ADHD for and which complaints are characteristic;
2. What complaints and problems of the client are possibly related to the ADHD (ask specifically for sleep problems, performance about training and work, relationship problems, financial problems, alcohol and drug abuse);
3. What are the thoughts about the causes of ADHD;
4. Relationship ADHD and aggressive behaviour;
5. Comorbid disorders – if present –: discuss the possible connection/interaction;
6. Options for treatment of ADHD directed to psychotropics, and practical support.

Explanation for ADHD:
ADHD is the abbreviation for: Attention Deficit Hyperactivity Disorder. In Dutch, it means: Attention disorder with hyperactivity. The main symptoms of ADHD are: concentration problems (also called inattention), hyperactivity and impulsivity.
There are three types of ADHD:
• ADHD; predominantly inattentive type. This is also called ADD. With this type, there are mainly problems with focus and concentration.
• ADHD; predominantly hyperactive-impulsive type. With this type, there are mainly problems with hyperactivity and impulsivity.
• ADHD; combined type. With this type, there are problems both with concentration as well as with hyperactivity and impulsivity. This joint type is most common.
With reference to treating ADHD (J.J.S. Kai).
How common is it?
In the Netherlands, ADHD affects 1-2.5% of the adult population. ADHD occurs often in families. ADHD is evident during the adult lifetime, with many complaints and dysfunction. It usually occurs comorbidly with anxiety, depression, addiction and personality disorders.
How can you explain ADHD?
Chemicals ensure the transfer of stimuli from one cell to another cell, in our brain. In this way, all cells are connected and they can communicate with each other. There are many different types of chemicals in our brain. With ADHD, it is suspected, that there is a relative lack of two substances that are important for inhibiting behaviour: dopamine and norepinephrine. This applies both to inhibiting movements (hyperactivity), impulses (impulsivity), associations (inattentiveness, chaos) as emotions (mood swings, irritability, temper outbursts). The medications for ADHD increase the amount of dopamine and norepinephrine (via inhibition of reuptake either via stimulation of the supply) in the spaces between the cells so that these can do their work better or can inhibit better, (Meijer, 2011).
ADHD (medication) and substance abuse
The effect of medication with alcohol and cannabis use cannot be properly assessed. Limit alcohol consumption to a maximum of 2 glasses on the weekend or at a party. Reduce cannabis to, a maximum, of 1 joint per day, after attempting to stop adjusting the medication. The concentrations problems of ADHD increase with cannabis so that the beneficial effects of stimulants on concentration is negated.

Si dovrebbero discutere i seguenti argomenti:
1. Che cosa significa “ADHD” e quali sono i sintomi caratteristici;
2. Quali sintomi e problemi del paziente possono essere associati con l’ADHD (chiedere specificamente dei problemi del sonno, dello stress dovuto a studio o lavoro, dei problemi sentimentali, di problemi finanziari, dell’abuso di alcol e droga);
3. Le possibili cause dell’ADHD;
4. Relazione tra l’ADHD ed il comportamento aggressivo;
5. Disturbi concomitanti (comorbidità) - se presenti: discutere le possibili connessioni/interazioni;
6. Opzioni per il trattamento dell'ADHD tramite farmaci psicotropi e sostegno pratico.

Informazioni relative all’ADHD:
ADHD è un acronimo dall’Inglese “Attention Deficit Hyperactivity Disorder”. In Italiano è chiamato Disturbo da Deficit di Attenzione / Iperattività. I sintomi principali dell’ADHD sono: problemi di concentrazione (chiamati anche disattenzione), iperattività e impulsività.
Ci sono tre tipi di ADHD:
• ADHD - variante con disattenzione predominante. Detta anche ADD. Pazienti con questa variante hanno principalmente problemi di attenzione e concentrazione.
• ADHD - variante con iperattività-impulsività predominante. I problemi principali sono l’iperattività e l’impulsività.
• ADHD - variante combinata. I pazienti con questa variante presentano problemi sia di concentrazione che di iperattività e impulsività. Quest’ultima è la variante più comune.

Quanto è comune?
Nei Paesi Bassi, l’ADHD colpisce 1-2,5% della popolazione adulta, spesso ricorre in famiglia e si manifesta in età adulta con vari disturbi e disfunzioni. Solitamente è affiancato da ansia, depressione, dipendenza e disturbi della personalità.
Quali sono le cause dell’ADHD?
Nel cervello, segnali chimici garantiscono il trasferimento di stimoli tra una cellula e l’altra. In questo modo, tutte le cellule sono connesse e possono comunicare tra loro. Nel nostro cervello ci sono diversi tipi di sostanze chimiche. Nei pazienti con ADHD, si sono postulati livelli relativi bassi di due sostanze che giocano un ruolo importante nella regolazione dell’autocontrollo: la dopamina e la noradrenalina. Questo riguarda sia il controllo del movimento (iperattività), degli impulsi (impulsività), dell’associazione (disattenzione, disordine) e delle emozioni (sbalzi d’umore, irritabilità e scoppi d’ira). I farmaci usati nel trattamento dell’ADHD (tramite l’inibizione della ricaptazione o lo stimolo della produzione) aumentano la quantità di dopamina e norepinefrina presente nello spazio tra le cellule affinché quest’ultime possano compiere meglio il loro lavoro e migliorare l’autocontrollo.
ADHD (terapia farmacologica) e abuso di sostanze
Non è possibile valutare adeguatamente l’effetto della terapia farmacologica in pazienti che fanno uso di alcol e cannabis. Si suggerisce di limitare il consumo di alcol ad un massimo di 2 bicchieri durante il fine settimana o ad una festa. Si raccomanda di ridurre l’uso della cannabis a non più di uno spinello al giorno, dopo aver tentato di interrompere variando il dosaggio del farmaco (NDT: can somebody please check the original Dutch, as the English translation doesn’t make much sense). I problemi di concentrazioni causati dall’ADHD aumentano con l’uso della cannabis, pertanto gli effetti benefici degli stimolanti sulla concentrazione sono eliminati.
